The Patient-Centered Medical Home (PCMH) is a concept that is trying to change the face of primary care. By practicing team-based care in the office and fostering a true partnership between patient and provider, the PCMH is hoping to help improve patients’ health and help reduce much of wasteful spending in American healthcare system.

FMGW has been working on implementing this model of care in our office. By following the seven principals in the PCMH (listed below), we strive to provide you with the best healthcare possible.

Seven Principals of the PCMH

  1. Personal Physician ~ having one doctor that coordinates all of your healthcare
  2. Physician directed medical practice ~ having a trained doctor over-seeing the office
  3. Whole person orientation ~ treating the entire person, not just one organ or system in the body
  4. Care is coordinated and/or integrated ~ multiple providers (i.e. nutritionists, psychologist…) coordinate patient care in a team based approach
  5. Quality and safety ~ reducing medical errors and providing evidence based care
  6. Enhanced access ~ not having long wait times to get in to see a provider
  7. Payment ~ making the concept financially viable

On November 30, 2008; FMGW obtained qualification from the National Committee on Quality Assurance as a Level-3 Physician Practice Connection- Patient Centered Medical Home. After completing a lengthy application, FMGW was able to show that they have the policies and producers in place to obtain this highest level of qualification.
